Healthier Neighbourhoods PCN

We are pleased to share the exciting news that Paxton Medical Group has been given approval to form a new Primary Care Network (PCN) with partner practices at Heatherlands Medical Centre, Prenton Medical Centre, and Woodchurch Medical Centre. 

This important development has been in the works for a number of months, and we’re delighted to be able to finally share it with you.

The new PCN will be called Healthier Neighbourhoods PCN, reflecting our commitment to deliver exceptional patient-centred care to our communities. There will be no loss of patient services.

Services will be continued or enhanced, and in most cases delivered closer to home. The benefits to our patients include:

• Services will be delivered in our patients’ local surgeries rather than elsewhere on the Wirral.
• Our patients will be able to access enhanced services such as physiotherapy, health coaching, and social prescribers closer to home.
• Patients will benefit from additional GP appointments, including evenings and weekends (launching later in 2024).
