Your Consent to Receive the Flu Vaccination

At our practice, flu vaccinations may be given by either a nurse or a suitably trained Healthcare Assistant (HCA).

By booking an appointment for the Flu Vaccination you are confirming that:

• You have read and understood the information provided about the flu vaccine.
• You have had the opportunity to consider whether you wish to receive the vaccination.
• You consent to receiving the flu vaccination at your appointment.

If you have any questions about the vaccine, would like more information, or are unsure whether the vaccination is right for you, please do not use the booking link. Instead, please contact the surgery and arrange an appointment with a GP or Practice Nurse, who will be happy to discuss the vaccination with you before you make a decision.

On the day of your appointment, the clinician administering the vaccine will confirm your identity, check that you are eligible for vaccination, and ensure that you are still happy to proceed. If you have any concerns at that time, please let the clinician know before the vaccine is given.
